Personal information on this form is collected under the authority of the Freedom of Information & Protection of Privacy Act (the Act) for administrative purposes of the District of Sicamous. Personal information is protected from unauthorized use and disclosure under the Act and may only be used and disclosed as provided by the Act. Questions about the collection of personal information can be directed to the chief administrative officer or freedom of information coordinator, District of Sicamous, PO Box 219, 446 Main Street, Sicamous, BC V0E 2V0, corporate@sicamous.ca, 250-836-2477. Additional Information

The fee for the installation of memorials is $100. Of this fee, $20 is contributed to the Cemetery Maintenance Care Fund. This application will be accepted once payment is made at the District Office (446 Main Street, Sicamous BC).

This permit is to be used in accordance with and subject to the District of Sicamous Cemetery Management Bylaw No. 773, 2009 and the District of Sicamous Fees and Charges Bylaw No. 740, 2009 that may be in effect at the time of purchase, or as they may be amended from time to time thereafter. Memorials are to be delivered to the District of Sicamous Public Works Department, 1450 Solsqua Sicamous Road, Sicamous, BC.

Memorials shall only be placed, installed or removed by the caretaker or other person duly authorized by the District of Sicamous.
